HOW TO BOOK YOUR VACATION

Reserving with me guarantees that you will be part of the Angela and Pierre's wedding group and ensures you will be on the same flight and in the same resort. Please e-mail your reservation request to me at dkeizer@tpi.ca. This ensures accurate information, and eliminates any issues like spelling mistakes in names, etc.

Each person booking must provide the following:

1. Your Full Name as it is listed in your passport. It is essential that your passport name and your airline ticket name be identical. If you have not yet requested a passport, make sure that the name you provide me is exactly the same name that you put on your passport application. If you find later that the name you gave me is not the same as what is listed in your passport, the airline will charge you $200 or more to change the name. If the name is not changed to reflect the exact same name as your passport, you will be denied boarding on travel day!

2. Your Full Mailing Address

3. Your Phone Number (s). I need at least one phone where you can be reached. I require this for a number of reasons: Should the resort sell out, they have the right to ask for final payment then instead of waiting for the final payment date....Should the airline change the flight times....Should there be any problem with the credit card you gave me for payment....I need to be able to reach you AT ONCE.

4. Your Email Address where I can reach you. I require your email address so that I can send out a blanket email should any group issues arise, or to provide general information and updates.

5. Your Date of Birth. I require your date of birth for my files, and in order to process your travel insurance.

6. Your Credit Card Information. I will require your credit card information for the initial deposit and final payment. If both payments will be made using the same credit card then I only require the one credit card number. I would like to suggest that you send me all of the information above - except your credit card details - via email. I will then contact you to collect your more personal information, like your credit card numbers. Transat requires a deposit of $250 per person by September 1 to hold the vacation package. Final payment will be required October 1, 2009. You can make your final payment at any time prior to this, but we will need final payments to be processed by that date.

TRAVEL PROTECTION - DON'T LEAVE HOME WITHOUT IT!

Here is a brief summary of the travel insurance plans that I recommend for your trip to protect your investment and provide you peace of mind while you are away.

RBC Deluxe Travel Insurance covers your emergency medical needs, doctor visits, tests, hospitalization, etc. This plan also covers cancellation and interruption insurance should you have to cancel due to your medical emergency or that of an immediate family member, or should you have to return home because of medical emergency. Trip delays are also covered as well as flight and travel accident, delayed or lost baggage or personal effects. Anyone over the age of 73 will be required to remit a medical questionnaire prior to booking to ensure the coverage will be permitted by RBC. All pre-existing medical conditions must be stable at least 90 days prior to booking.

RBC Non Medical Travel Insurance covers cancellation, interruption and trip delays, flight accident, travel accident, delayed baggage and personal effects and damaged baggage. There is no medical coverage included.

All insurance policies will be booked at time of booking your vacation. The cost of insurance will be processed at the time of reservation.

PASSPORTS AND VACCINATIONS

You must have a valid Canadian passport for all trips outside Canada. A passport is the only reliable and universally accepted identification document, and it proves that you have a right to return to Canada. Your passport must be valid one month beyond your departure date from Cuba. Passport applications can be made on line at http://www.voyage.gc.ca/, or picked up at a Canada Post Office.

It's a good idea to visit a health care professional or clinic six to eight weeks before you travel. Hepatitis A and B shots may be recommended, and you should be sure that your regular inoculations (e.g., tetanus, diphtheria, polio, and measles) are up to date.
